How to Order
In a securely packed box, send us your tape(s) through a registered mail service such as Fedex, UPS or registered US Mail. (NOTE: Keep your shipping documentation and tracking numbers)
Fill out the form below as completely as possible, print it out and enclose it in your package.
Also include a personal check made out to "Preserve Your Videos" for the estimated cost of the services to be performed. See the "Price List" to the left to calculate this amount.
Note: If the actual cost exceeds the estimate, you will be billed for the balance. If the actual cost is less than the estimate, we will enclose a refund check with your order.
